In California, we value wellness, vitality, and living life to the fullest. Yet, from the tech-fueled demands of Silicon Valley to the high-energy lifestyle of Los Angeles, our constant screen time, late-night habits, and demanding schedules frequently disrupt our internal clocks. Getting up early for morning meetings on the East Coast makes things even worse. We are sleep-deprived, and it is well-established that sleep-deprived people gain more weight.

At sleepandobesitymed.com, we look at your health through the lens of modern biology. As a specialized, direct-access virtual practice, we help California residents master this biology to achieve effortless sleep and sustainable weight management.

The Science of Circadian Disruption & Weight Gain

Your body runs on a strict 24-hour clock regulated by light and darkness. When this clock is disrupted by poor sleep, late nights, early mornings, or insomnia, it throws your metabolic hormones into chaos:

  • Insulin Resistance: Sleep-deprived cells become sluggish, storing glucose as stubborn fat rather than burning it for fuel.
  • Melatonin Suppression: Artificial blue light from our devices halts melatonin production, which not only delays sleep but also impairs nighttime fat metabolism.
  • The Midnight Craving Cycle: When sleep is short, the brain craves high-calorie, fast-acting energy sources to stay awake, sabotaging your best nutritional intentions.

By addressing sleep quality and circadian alignment, we lay the biological foundation for your body to shed weight naturally.

My Intimate Ties to California

My connection to the Golden State is deeply personal. I first visited California in 1979, starting in Los Angeles. (People were mostly slim back then in California!) Disneyland and Universal Studios were awesome, and I was thoroughly enamored by the Hollywood sign and the stars along the Hollywood Walk of Fame. My first girlfriend was from California; her father owned an almond farm, and she attended medical school in Southern California.

The drive up to San Francisco was absolutely fascinating. The beach at Carmel-by-the-Sea held me spellbound—it was there that I saw seaweed for the first very first time. As an avid reader, exploring Monterey and its rich connections to John Steinbeck, one of my favorite authors, kept me enthralled. San Francisco, with its incredibly steep streets, historic cable cars, and the majestic Golden Gate Bridge, was an absolute treat, and I fell deeply in love with the towering redwoods of Muir Woods National Monument.

Later in my career, I almost ended up at Stanford for my final residency year, though I ultimately chose to stay at Johns Hopkins in Baltimore. Years later, a mentor in San Francisco topped up my training in EEGs right before I took my Clinical Neurophysiology Boards. Today, my favorite son (I only have one!) and his family live in Silicon Valley, giving me the best reason of all to keep my ties to California strong.

Sleep and Obesity Telemedicine in California: services tailored to your lifestyle

CPAP Alternatives for Active Lifestyles: CPAP machines can be cumbersome. We focus on excess weight loss to improve your obstructive sleep apnea (OSA) and resultant sleepiness. Excess weight loss also improves cardio-metabolic health—something that evidence increasingly shows CPAP does not improve. In fact, for low-risk OSA patients, CPAP may even worsen cardiovascular outcomes. However, because of the daytime sleepiness and fatigue caused by sleep apnea, even low-risk patients still require highly effective treatment.

Medical Weight Loss & Metabolic Therapy: We look beyond fad diets. Dr. Sangal provides science-based weight management strategies, including the evaluation and management of modern peptide therapies (GLP-1s) to help balance your hunger hormones, paired with structured lifestyle management. This combined medical approach helps you achieve the superior results seen in clinical trials rather than the poorer results generally seen in the real world.
